will file your claim for you but please note that you are responsible for any fees not covered by insurance.
Co-Pays are due at the time of the service.
If your insurance requires a referral, it is your responsibility to obtain it prior to your appointment. Please check with your insurance plan before your appointment.
You must bring your insurance card and photo identification to your doctors appointment. If you fail to do so, you will need to pay in full at the time of the service.
